H1N1 (swine) flu is a new flu virus that has been causing many people to become ill around the world.

Newton H1N1 Vaccine Clinics:

NEW The Health & Human Services Department has both seasonal and H1N1 flu vaccine available for children and adults. If you are interested in receiving an H1N1 or seasonal flu vaccine, please call the main health department number at 617-796-1420 to leave your name and phone number. A public health nurse will make an appointment for you to receive your vaccine at the health department, 1294 Centre St. in Newton Centre.

Attending a Newton H1N1 Flu Clinic?
Print these two forms, fill them out, and bring them with you.
A set of forms is needed for each person being vaccinated.

People attending a Newton Flu Clinic for H1N1 should print and complete the Vaccine Administration Record and the H1N1 Flu Vaccination Screening Form and bring both forms to the clinic
