- Building Permit Application form, filled out completely
- Photocopies of all pertinent licensing
- Insurance Affidavit/Certificate
- Debris Removal Form
- Official notice of disconnection of all utilities (as applicable)
- Site Plan of lot with existing structure, showing existing grades (topography)
- Water, sewer, electric, gas, telephone, cable, etc.
- Proof of removal or absence of hazardous materials (see Health Dept.) (as applicable)
Demolition of greater than 50% of existing bldg./structure
- Is bldg. older than 50 years or located within a Historic District (see Historic Commission requirements)
- Movement of equipment across City property - Sidewalk Bond (see Public Works)
- Safe guards to site access during demolition
- Dust/Particulate matter abatement (Health Dept.)
